Here you can download a template to create your Healthy Schools Action Plan. The template is available to download either as Word document or a PDF. If you would like to complete the template online we would suggest using the Word document version, but if you would like to print it and fill it out by hand we would suggest using the PDF version.

The template has been created to ensure the actions you wish to take to become a healthier school are SMART (specific, measurable, achievable, realistic and time-bound).

We would suggest that you choose three of the Healthy Schools criteria that you have marked amber to focus on initially. You can use the Areas for Action section of the portal to help you decide which criteria you want to focus on.
